Organizations may structure query retention differently depending on legal requirements, health-record policy, technology, and organizational governance. Queries may be part of the permanent health record or retained as another retrievable business record, depending on policy. The key point is that disposition should not be manipulated to hide unanswered or inconvenient clarification activity.

Destroying unresolved queries can impair auditability, create an appearance that the organization is selectively preserving only favorable documentation activity, and undermine the integrity of the CDI process.

The 2026 ACDIS/AHIMA guidelines are the current foundational standard for query-practice design and supersede prior versions.

A query need not automatically remain in the permanent medical record forever; retention requirements depend on applicable organizational and legal policies. What is unacceptable is an inconsistent approach designed to remove evidence based on outcome.

The CDI specialist should first review the final coded record together with the Coding Clinic guidance identified by the coder. This provides an objective basis for understanding why the final MS-DRG differs from the CDI working DRG.

A mismatch does not automatically indicate that either CDI or coding made an error. Working DRGs are provisional and may change after complete review of discharge documentation, operative reports, pathology, final diagnostic information, coding conventions, official guidelines, or authoritative Coding Clinic advice.

Reviewing the applicable guidance before contacting the coder promotes an informed and efficient reconciliation process. Once the specialist understands the coding rule, discussion with the coder may still be useful if ambiguity remains. Immediately escalating to the coding manager is unnecessary unless the issue cannot be resolved through normal collaboration. Forwarding the record without investigating the mismatch would defeat the purpose of a reconciliation process and could allow recurring education opportunities to remain unidentified.

Mismatch analysis is a valuable CDI quality metric because it can identify documentation gaps, coding-knowledge issues, changes in authoritative guidance, and opportunities to improve CDI/coding alignment.

The findings are characteristic of a large or massive pleural effusion. Pleural fluid produces dullness to percussion and diminished breath sounds because fluid within the pleural space reduces normal transmission of respiratory sounds and replaces the normally air-filled acoustic environment.

A sufficiently large effusion can cause complete or near-complete opacification of a hemithorax on chest radiography. Importantly, a large-volume effusion exerts mass effect and can push the mediastinum away from the affected side, producing contralateral mediastinal shift. That combination strongly supports pleural effusion as the appropriate diagnostic clarification.

Pneumonia can produce focal opacity and abnormal breath sounds, but extensive consolidation often causes air bronchograms and typically does not create the same fluid-related contralateral mass effect. Atelectasis can opacify a hemithorax but usually pulls the mediastinum toward the affected side because of volume loss. Bronchiectasis has different radiographic characteristics, including chronic bronchial dilation. Heart failure may cause pleural effusions but does not itself describe the immediate radiographic abnormality being presented.

A CDI query should seek provider confirmation rather than independently diagnose the effusion.

A high volume of unanswered queries most directly indicates inadequate provider engagement. This question is adapted from the current ACDIS Candidate Handbook's publicly released CCDS sample questions.

A provider may legitimately agree or disagree with a CDI query. Disagreement does not necessarily indicate poor engagement because the purpose of a query is clarification, not obtaining a predetermined diagnosis. A physician who reviews the clinical information and documents that the condition is ruled out, clinically undetermined, or otherwise different from the proposed possibilities is still participating appropriately.

Likewise, the number of queries generated is primarily a CDI workload or opportunity metric. It does not demonstrate whether physicians are responding.

A persistent nonresponse rate is more concerning because unanswered queries can delay final coding, create unresolved ambiguity, affect billing timeliness, reduce documentation completeness, and indicate weaknesses in provider communication or organizational query escalation procedures.

A CDI manager should not evaluate engagement from a single metric in isolation. Response time, response rate, education participation, documentation trends, service-line differences, and escalation frequency provide additional useful context.
